If you are taking part in the animal experience, please fill-in the form below to complete your experience booking.

You will need to complete one of these forms for each participant on the experience.

 

Please note, if this form has not been completed, we may not be able to deliver your experience on your chosen date.

Does the participant have any phobias or allergies?
Does the participant have any visual, hearing or mobility impairments?
Does the participant have any additional learning difficulties or disabilities?
Does the participant have any other medical conditions that we need to be aware of which may put them, the animals or zoo staff at risk?
Is the participant pregnant?
Is the participant physically fit and able to walk across uneven ground?
